At SRMG our doctors work with you to navigate through your healthcare issues and can provide advice about additional services which you can access to help you manage in your own home.

Do you perform annual Health Assessments for patients aged over 75 years at SRMG?

We offer a comprehensive annual health review for our patients aged over 75 years. This represents an opportunity to consider areas of your health that may not have recently been covered during routine medical appointments. It involves a practice nurse performing a comprehensive interview which takes around 1 hour. For patients who are unable to attend the practice for health reasons, a nurse will attend your home to conduct the assessment if you are located within a 10km radius of the practice. Afterwards, you attend your regular GP for a medical check-up and completion of the assessment.

Specifically, the assessment reviews your physical ability, social support and psychological well-being. The focus of Health Assessments is preventative care – solving problems before they happen, and identifying things that can be done to help you live independently in your own home for as long as possible. At SRMG, we bulk bill the Over 75 Health Assessment so there is no cost to you.

Do you provide care at Residential Aged Care Facilities?

We offer care to residents at all the Residential Aged Care Facilities in Albany. If you have a regular doctor at SRMG they will continue to provide care for you at your facility, we are also happy to accept new patients who may not already have a relationship with a GP in Albany.

Our Nurse Practitioner, Jemma Mustey, has a special interest in Aged Care. She visits SRMG patients in aged care facilities in Albany on a weekly basis and works closely with you, your doctor and the staff at your facility to ensure your ongoing health needs are met.
